Health insurance can be confusing to navigate, even for those who are experienced. Let us simplify the beginning of the claims process by taking you through a few key recommendations.

You don’t want to have to use your insurance, but in case of an unexpected illness or accident, having that safety net creates invaluable financial peace of mind. In case you do need to use your Platinum-marketed supplemental insurance policy, here are some suggested tips for a smoother claims process.

Call Platinum’s Customer Service Team

Before you do anything else, we recommend that you call our friendly and knowledgeable customer service team at 1-855-637-6550. We can answer any questions, ensure that you’re submitting accurate information, put your claim on record and more.

Gather the Information You Need

After you’ve called Platinum customer service, you’ll need to request UB-04 and CMS-1500 forms from your medical provider. Keep in mind that a balance-due statement from the provider is not enough. If you’re unable to obtain the UB-04 and CMS-1500 forms, you can submit an itemized bill with the following information:

  • Patient’s name
  • Medical provider’s name, address and tax identification number
  • Procedure and diagnosis codes
  • Date of service
  • Charges incurred
